January 2023, throughout California (with specific Managed Medi-Cal Plans) “Nursing Facility Transition / Diversion to RCFE / Home” was launched. With it, we can help you and your team safely-and-successfully discharge patients who may otherwise have limited financial and/or care options.
This is not the ALW program
Under this new program, we have access to a large database of RCFE and other providers, as well as pathways to pay for care that were previously unavailable.
